For death certificate
You'll need to provide:

• deceased's NRIC, FIN, or passport number (as per what the doctor used during death certification)

• death document number

• date of death

For stillbirth certificate
You'll need to provide:

• mother's NRIC, FIN, or passport number (as per what the doctor used during death certification)

• stillbirth document number

• date of stillbirth

These information will be given to you by the hospital or doctor.
