How do I make changes (e.g. changes to appointed Donee) to my registered LPA?
To make changes to your appointed Donee (including adding, removing or changing a Donee) for a registered LPA, you will need to revoke your existing LPA and make a new one.
Before revoking your LPA, you must inform all your Donees in writing about your decision. You can notify them either by:
delivering a written notice in person
post
email
To revoke your existing LPA, log in to Office of the Public Guardian Online (OPGO) using your Singpass, to submit your notice of revocation.
Select Revoke LPA under your registered LPA.
Review and acknowledge the information provided.
Complete the required declarations.
Make payment.
Submit your notice of revocation.
If you are unable to access OPGO, submit your completed hardcopy revocation form to the Office of the Public Guardian by post, or via FormSG at https://go.gov.sg/opg-enquiry.
The Public Guardian may require you to provide further information or documents before revoking your registered LPA.
You may start drafting a new LPA while waiting for the cancellation of your existing LPA. However, the new LPA will only be accepted by OPG for registration after your existing LPA is cancelled. (adhered to copy in link)
Before submitting your new LPA for registration, you will need to have it certified by a Certificate Issuer.
