I have signed up for the free Lasting Power of Attorney and Advance Care Planning (LPA-ACP) mobile clinic session, what do I need to prepare?

Please bring along your NRIC and CHAS Blue or Orange card for identification purposes.

For LPA, please complete your LPA form and ensure all your Donee(s) and Replacement Donee (if any) have accepted their appointment. You should have received an SMS/email notification to visit the certificate issuer. Your LPA status should be “Pending Certification”. Your Donees do not have to be present on that day, unless they are your Nominated Healthcare Spokespersons for the ACP.

For ACP, please prepare for the session by reflecting on the values and preferences that are important to you using an ACP Booklet or the LPA-ACP Tool on My Legacy (https://mylegacy.life.gov.sg/find-a-service/lpa-acp/). Your Nominated Healthcare Spokesperson(s) will have to be present for the session.
