How do I apply for a free Lasting Power of Attorney and Advance Care Planning (LPA-ACP) mobile clinic session?

Thank you for your interest in the Lasting Power of Attorney - Advance Care Planning (LPA-ACP) Mobile Clinics. The mobile clinics are open to only Singaporeans aged 50 years and above with a valid CHAS Blue/Orange card. 

If you meet our eligibility criteria and would like to register for our upcoming mobile clinics, you can visit our website - https://www.giftofcertainty.gov.sg/events/. We will open the appointment slots for the mobile clinics for booking about 3 weeks before the event. There will be a "Book a Slot" button at the bottom of the event page once booking slots are open. Kindly note that all bookings can only be made through our website.

If you do not meet the eligibility criteria above or wish to complete your LPA and ACP sooner, you can do so by contacting an LPA Certificate Issuer and ACP Facilitator. You can refer to this page - https://mylegacy.life.gov.sg/find-a-service/ to find a service provider. Do check with your preferred service provider on their charges and availability before heading down. 

For ACP, if you are receiving medical care at a public healthcare institution, you can ask your care team to arrange an ACP session for you. If you are not receiving treatment at any public healthcare institution, you can contact a community ACP facilitator using the link above.
