Can I submit a hardcopy Lasting Power of Attorney (LPA) application?

All LPA applications should be submitted online. If you're uncomfortable with technology and need help completing your LPA application online, visit a ServiceSG centre or contact the Office of the Public Guardian (OPG).

The Office of the Public Guardian (OPG) may accept hardcopy LPA applications if you fall within one of the following exceptional situations:

  • you or your Donees (including replacement Donee) are unable to complete the application online due to physical disability or other circumstances

  • the prepare your LPA service on My Legacy and the Office of the Public Guardian (OPGO) is down

  • you or your Donees (including Replacement Donee) do not have an electronic device to receive SMSes or to create and access the Singpass account

  • you or your Donees (including Replacement Donee) are not eligible for a Singpass account

If you are unable to obtain a spare smart device, you may submit a hardcopy LPA form instead. Mail in or drop off the completed hardcopy LPA application to the Office of the Public Guardian (OPG) at 3 Bishan Place #03-00, CPF Bishan Building, Singapore 579838.
