You may disclaim your appointment as a Donee or Replacement Donee in a registered Lasting Power of Attorney (LPA) by completing the Disclaimer Form (DN1) on OPG’s website.

You must give notice to the Donor and any remaining Donee(s) appointed that you wish to disclaim your appointment by:

i. Completing a separate Disclaimer Notice Form (DN2) available on OPG’s website; and

ii. Sending the completed and signed Disclaimer Notice Form (DN2) to the Donor and every other Donee (if any) by delivering it personally, sending it by post, or if the postal address is not known, via email.

Please submit the completed Disclaimer Form (DN1) to OPG’s office by post or email.
