What information do I need to prepare before making an incident report to MOM?
The following information are required when reporting an incident to MOM:
- Informant's personal particulars and organisation details, where applicable.
- Accident details, e.g. when, where and how did the incident happen.
- Occupational disease details.
- Injured person's details, e.g. personal particulars, employment, nature of injury, insurance policy details and other pre-existing accident condition information.
