How can I get a referral for subsidised care at public SOCs?

A referral for subsidised care at public SOCs can be obtained from either CHAS clinics or polyclinics. Do note that you will need to be a PG/MG/CHAS cardholder to obtain subsidised SOC referrals from CHAS clinics.
