Does the nursing home charge for performing the Resident Assessment Form (RAF) for the applicant?

Nursing homes do not charge for performing the RAF for their residents. If the individual resides in a nursing home and would like to undergo the RAF in place of a disability assessment conducted by an MOH-accredited severe disability assessor, please approach the individual’s nursing home for further assistance.
