I have been issued a Service Injury (SI) Card previously but the doctor said the diagnosis of my injury has changed. What should I do?

As there is a change of diagnosis to your injury, please obtain a memo from your Specialist to explain your current medical condition. The memo should also certify if your current medical condition is due to the incident leading to your service injury.  You may email the memo to the MINDEF/SAF HR Shared Services Centre (HRSSC) at DisabilityMgt@defence.gov.sg for us to process the change in your diagnosis. Once the new diagnosis is approved, a new Service Injury card will be issued and sent to your residential address.
