I encountered an error when using my Service Injury Card at the polyclinics/ government hospitals. What should I do?
Please inform the clinic/hospital staff to access your service injury records using the Medical Benefit Identification Memo (MBIM) number that is indicated on your Service Injury Card. It is a serial number on the top right-hand corner of your card, starting with the prefix "MC", followed by 9 digits.
