How can I request a replacement of the Service Injury (SI) Card?

Please submit your request with indication of your name, NRIC number, email and residential address to MINDEF/SAF HR Shared Services Centre (HRSSC) at InjuryMgt@defence.gov.sg. A digital and physical Service Injury Card will be sent to you at the email and residential addresses that you have provided within 2 weeks.
