Do I need to print the completed and endorsed medical questionnaire?
If your endorser had endorsed the medical questionnaire online, you do not need to print it out. However, if you have opted for your medical questionnaire to be endorsed via hardcopy, you will need to print out the completed medical questionnaire from your OneNS dashboard and get it signed by your endorser before submitting it on the day of your pre-enlistment medical screening.
