What does my parent need to do to endorse the medical screening questionnaire online?

Your parent or guardian will need to endorse the following sections of the questionnaire: Drug Allergy & G6PD Deficiency, Personal Medical History and Family History. Please ensure that the person you have indicated to endorse the form has full knowledge of your medical history. You will be required to provide his/her email address and contact number so that we can notify him/her to endorse the questionnaire.
