Your Healthier SG GP clinic's care team will facilitate your referrals to specialist outpatient clinics (SOC) and/or Hospital as part of your care (if required). Do inform your clinic's team if you have a preferred SOC and/or Hospital.

There may be situations where you may be required to provide information or submit documents to complete the referral. Your Healthier SG GP clinic's care team will advise you if you need to fill up documents as part of the referral process.
