- You will receive the same level of care regardless of whether you choose to see a doctor at a GP clinic or Polyclinic as there are standard care protocols to guide doctors in managing chronic conditions such as diabetes, hypertension and lipid disorders. The protocols will also cover health screenings, medications, lifestyle adjustments, and referrals to specialist and acute care when necessary. Healthier SG GP clinics, like polyclinics, will be supported by a healthcare team managed by their Primary Care Network. We recommend that you choose a GP clinic that is convenient for you and whom you have been seeing over the years for better rapport and trust.

i. Screenings and vaccinations
Benefit from fully-subsidised nationally-recommended screenings and vaccinations such as influenza shots, based on your age, health conditions and vaccination history, for Singapore Citizens.
Early detection and treatment can help prevent or delay the onset of common chronic conditions and cancers such as diabetes, high blood pressure and cholesterol, colorectal cancer, breast cancer, and cervical cancer.

ii. Access to referrals for subsidised specialist care
Get referrals to public specialist outpatient clinics (SOC) for CHAS^, Merdeka Generation (MG) and Pioneer Generation (PG) cardholders at all Healthier SG Clinics. Receive affordable specialist care, without having to visit polyclinics for referral.
iii. Health Plan follow-up
Start your health journey with a fully-subsidised Health Plan consultation at your first visit to your Healthier SG clinic, and ensure good progress with free annual check-ins, either by phone or during your other medical appointments*.
*Prevailing fees for services outside Healthier SG scope will apply.
iv. Out-of-pocket payments
From early 2024, fully pay with MediSave for treatment of chronic conditions under the Chronic Disease Management Programme (CDMP) without cash co-payment, up to the MediSave withdrawal limit.
v. Subsidies for selected chronic medications

How will I know if I am eligible for Healthier SG?
Singapore Citizens (SC) and Permanent Residents (PR) can enrol in Healthier SG. The rollout phases will prioritise those who are more likely to need medical care or suffer from chronic illnesses.
MOH is progressively inviting Singapore Citizens and Permanent Residents aged 60 years and above via SMS to enrol with a Healthier SG clinic or polyclinic. SMS invitations will continue to be sent to residents when they turn 40 years old.
What are the chronic conditions covered under Healthier SG Chronic Tier?
Enrolees with any chronic conditions under the Chronic Disease Management Programme (CDMP) and have CHAS/Merdeka Generation/Pioneer Generation cards can opt for the Healthier SG Chronic Tier, which allows them access to common chronic drugs at prices comparable to polyclinics. What is Healthier SG?
Healthier SG is a national strategy to transform Singapore's healthcare system to focus more on preventive care to help residents stay healthy, detect health issues early and manage them well. Under Healthier SG, residents will be empowered to take steps to improve their health proactively with the support of their trusted family clinic and community partners.
If I have a Civil Service Card (CSC), will I still be entitled to Civil Service medical benefits if I choose to enrol with a GP instead of a polyclinic?
There is no change to a civil servant’s Civil Service medical benefits when he/she is enrolled in Healthier SG.
The annual claim limits, per-visit claim limits and list of subsidised items remain as per the individuals’ prevailing medical schemes. For example, civil servants and pensioners will continue to be subsidised for medical bills incurred at private clinics up to $20/visit, subject to the prevailing co-payment rates and annual claim limits (if any) based on their medical scheme.

What are the fully subsidised nationally recommended screenings and vaccinations? How am I eligible?
Nationally recommended screenings listed on the Screen-For-Life (SFL) programme, and vaccinations on the National Adult Immunisation Schedule (NAIS) are fully-subsidised for Healthier SG enrollees who are Singapore Citizens, when they visit their Healthier SG enrolled clinics.
Screen For Life (SFL)
The SFL programme covers nationally recommended screening tests for cardiovascular disease (diabetes, high blood pressure, high blood cholesterol), cervical cancer, colorectal cancer, and breast cancer.
National Adult Immunisation Schedule (NAIS)
The NAIS covers nationally recommended vaccinations such as pneumococcal and annual flu vaccinations for seniors aged 65 and above.

Can I enjoy subsidies for Healthier SG Chronic Tier, if I enrol with a polyclinic rather than a GP clinic?

There is no change to the prevailing subsidies at polyclinics.
Why should Pioneer Generation (PG) and Merdeka Generation (MG) cardholders still apply for CHAS, when they enrol into Healthier SG? What is the Healthier SG Chronic Tier benefits for them?
From early 2024, PG and MG seniors enrolled with the Healthier SG GP clinics can access the new Healthier SG Chronic Tier subsidy framework, where they can enjoy access to selected chronic medications, similar to those prescribed and subsidised at polyclinics, at prices more comparable to polyclinics.
Under this framework, PG and MG seniors can receive 87.5% and 81.25% subsidy (with no dollar cap) respectively for selected chronic medications. They will also receive subsidies of up to $360 per year for other components of their chronic care, such as consultation, lab tests and medications that are not part of the list of selected chronic medications.
Patients who are more likely to benefit from the new Healthier SG Chronic Tier are those with chronic conditions with higher medication needs and bills which exceed the current CHAS annual subsidy limits. Hence, patients are encouraged to consult their family doctors if they are suitable to switch to the selected chronic medications that are eligible for percentage subsidies under the Healthier SG Chronic Tier.
To cater for possible changes to their medical needs, patients will be allowed to switch between the original CHAS Chronic Tier and the new Healthier SG Chronic Tier. Should there be a switch between subsidy frameworks, the annual dollar subsidy balance will be pro-rated accordingly.
PG and MG seniors are nonetheless encouraged to sign up for the CHAS card as MOH continues to review enrolment benefits under Healthier SG.

Understand that the Enhanced MEDISAVE500 / 700 Scheme under Healthier SG initiative does not include outpatient clinics. Why are hospitals not included?
Healthier SG benefits and subsidies are intended to encourage residents to develop a long-term, trusted relationship with their regular family doctor, who would serve as the first point-of-contact to holistically manage their health, including any chronic conditions. As such, these benefits will be available at the resident’s enrolled Healthier SG clinic.
What is the price of vaccination at the polyclinics for adults aged 18 and above? And what am I eligible for?
For eligible Singapore Citizens who had selected a polyclinic as their Healthier SG clinic, they can receive fully-subsidised ($0) vaccinations for those listed under the National Adult Immunisation Schedule (NAIS), at their enrolled Healthier SG clinic. Individuals need to meet the eligibility criteria for these NAIS vaccinations.
For non-Healthier SG clinic enrollees, the total bill is based on a means-tested framework and may differ for each individual. If you prefer to get your vaccination at a polyclinic, please enquire directly with them on the estimated bill size.
