My spouse is incarcerated and/or mentally incapacitated. How should he access and complete the LifeSG subsidy application form?
During your completion of the LifeSG form, it will prompt you to declare your spouse’s circumstances. Once declared, the form will be adjusted such that it will not prompt you to route the form to your spouse. You may then proceed to complete the form and provide the necessary consent.
