You should inform the preschool so that the preschool can trigger the LifeSG form using your details. During your completion of the LifeSG form, it will prompt you to declare your spouse’s circumstances. Once declared, the form will be adjusted such that it will not prompt you to route the form to your spouse. You may then proceed to complete the form and provide the necessary consent.
