What should I do if my Certificate of Origin (CO) is torn, stolen or misplaced?
You (the exporter) may apply for a certified true copy of the Certificate of Origin (CO) with Singapore Customs, with the following documents:
• A written request using your company's letterhead specifying the reason(s) for the application. In the letter, you are required to indicate the CO's reference number, the exact copy(ies) of the CO that was torn, stolen or misplaced, and confirm that the CO was not returned to Singapore Customs for cancellation.
• A fresh set of CO, typewritten, with full details provided (including the CO's reference number) as per the torn, stolen or misplaced CO. You may obtain a new CO form from CrimsonLogic Service Bureau at #02-01 Tanjong Pagar Complex.
• Any copy(ies) of the CO that have not been torn, stolen or misplaced.
