You are required to submit the Annual Declaration for Anticipated Activities (ADAA) or Annual Declaration for Past Activities (ADPA) forms. The general information required includes the following: Name of company; Unique Entity Number (UEN); and Information of Declarant. Additional information will be required when you submit the annual report if the company is involved in the production, processing or consumption activities of CWC chemicals or the company is involved in the import or export of scheduled chemicals.
