How do I revoke the authorisation for the use of MediSave savings for my own and/or family member(s)' medical expenses?
The steps to revoke your authorisation for the use of MediSave savings for your own and/or family member(s)’ medical expenses depend on the type of Medical Claims Authorisation Form (MCAF) that you had previously signed:
If you signed the Medical Claims Authorisation Form (Single Institution) [MCAF(S)]
Please approach the Medical Institution where you signed the form to revoke your authorisation.

If you signed the Medical Claims Authorisation Form (Multiple Institutions) [MCAF(M)]
You may obtain a copy of the revocation form from any Public Healthcare Institution. Alternatively, you may download the revocation form and email the completed form to mohh.mcaf@mohh.com.sg.

Important:
Revoking your MCAF(M) authorisation revokes only the authorisation provided under the MCAF(M). Any MCAF(S) authorisations separately provided to individual Medical Institutions will remain valid and must be revoked directly with the respective Medical Institution(s).
