Make an appointment to visit any of our CPF Service Centre and bring along the following original documents:

  1. Medical report* from an accredited doctor stating that the member lacks mental capacity permanently;
  2. Lasting Power of Attorney or Court Order;
  3. Trust bank account statement or passbook; and
  4. Your NRIC.
*If the original medical report has been submitted to the Court, you can retrieve a copy via IFAMS and note that fees may apply for the request:

  1. Click on “Mental Capacity Act Application”
  2. Click on the tab “Other Matters”
  3. Click on “File Other Applications or Requests”
  4. Log in via SingPass (when prompted) as Individuals
  5. Select “File request for copy of documents” and fill up the required information.
Please ensure that you have all the necessary documents so that you can receive your loved one’s CPF savings smoothly.
