I would like to submit a medical report to support my Home Protection Scheme application. What is the information required in the medical report?

You may submit a medical report to support your Home Protection Scheme application, at your own cost. Your report must be up-to-date from a medical institution in Singapore and include the following information:
 
  1. Diagnosis and diagnosis date
  2. Reason(s) of hospitalisation, if any
  3. Details of treatment(s)
  4. Date(s) and details of surgery, if any
  5. Details of complications, if any, and treatment rendered
  6. Current condition and treatment(s)
  7. Investigation test results
