The ICCP teams within the following 25 subregions will begin managing referrals for clients belonging to their respective subregions from 28 September 2026 onwards:

• Bedok 2, 5, 6, 7

• Bukit Batok 2

• Bukit Merah 1, 2, 3, 4

• Bukit Timah 3, 4

• Clementi 2

• Geylang 1, 2, 3

• Jurong West 4

• Kallang 3, 5

• Marine Parade 1

• Sembawang 1

• Toa Payoh 1, 3

• Woodlands 1

• Yishun 1

What this means for referrals:

1. Community-up referrals that require the following services for these subregions are now handled entirely by the ICCP teams of these subregions.

   • Day Care

   • Dementia Day Care

   • Centre-Based Nursing

   • Day Rehab (PT/OT)

   • Day Rehab (ST)

   • Home Medical

   • Home Nursing

   • Home Rehab (PT/OT/ST)

   • Home Environment Review

   • Home-based Exercise Training

   • Enhanced Home Personal Care

   • Meals on Wheels

   • Medical Escort Transport

2. Community-up referrals for subregions not yet live continue to be handled by AIC, as before.

3. Hospital-down referrals continue to start with AIC in every subregion, who completes the care plan, interRAI, and initial service provider assignment.

   • For these 25 subregions, AIC then hands over to ICCP, which takes over managing the referral through to admission.

   • For subregions not yet live, AIC continues to manage the referral through to admission, as there is no live ICCP subregion to hand over to yet.
