How do I fill in the "Referral Institution" section?

What is this for?

Your referral will be sent to different AIC or ICCP teams depending on the institution selected. These questions help the system route it correctly. They have no impact on the referral's outcome.

Which category should I select?

Select the category that best matches your institution. If you're raising this referral on behalf of another team, select the category that matches their institution.

Which institution should I select?

Select your own institution. If you're raising this referral on behalf of another team, select their institution. If you cannot find your institution in the list, select "Others" as your category.

Which department should I select?

Select the department that requested this referral for the patient - not the service the patient is being referred for.

I'm an AIC staff / MSW creating a referral on behalf of a hospital team. What do I select?

Select the institution and department that requested this referral — i.e. the hospital department that identified the client's need and asked you to raise the referral.

E.g. If the referral request came from Singapore General Hospital's inpatient ward, select:

  • Category: Healthcare or residential institution

  • Institution: Singapore General Hospital

  • Department: Inpatient / Residential

I'm a hospital staff from a team that provides outpatient services. What do I select?

Select Healthcare or residential institution, then choose your institution and pick the most relevant department.

E.g. If you are from St. Andrew's Community Hospital's Day Rehabilitation Centre, select:

  • Category: Healthcare or residential institution

  • Institution: St. Andrew's Community Hospital

  • Department: Other community services

What happens if I select wrongly?

The receiving team can manually reroute the referral to the correct OIC if needed. Note that your selection is locked once the referral is submitted.
