This includes people who are:
Healthy
Living with a health condition
No matter who you are, you are likely to have values and preferences for how you want to be treated. It is important to think about these preferences and start planning for it early.
ACP is particularly important for people who are living with a health condition. You can consult your regular doctor, especially if you have chronic conditions (eg diabetes, heart disease, or chronic kidney disease) or any other medical concerns.
*For individuals below 21 years old and living with a health condition, please approach your healthcare team if you are interested to find out more about ACP.
