Care recipient's nationality and age

He/ she must be a Singapore Citizen or PR aged 30 and above.

Care recipient's MediSave Account balance

He/ she must have a MediSave Account balance of at least $5,000.

If the care recipient is tapping on his/ her spouse’s MediSave Account to top up the withdrawal amount, the spouse’s MediSave Account balance must also have at least $5,000.

Care recipient's needs

He/ she must:

• Require full assistance with at least three of the six activities of daily living. The six activities are:

  

• Undergo severe disability assessment.

  If he/ she is not staying in a nursing home

  • The assessment must be done by an MOH-accredited severe disability assessor.

  • Find out how to book an appointment to do the assessment.

  • You will need to pay the assessment fee first. It will be reimbursed if he/ she is assessed to be severely disabled. Read about the cost here.

  If he/ she is a nursing home resident

  • You can ask the nursing home to help submit a Resident’s Assessment Form (RAF) instead.

  If he/ she lacks mental capacity
