Day Hospice Care supports patients with serious illnesses, like advanced dementia or cancer, with palliative care and social activities at a centre. It also supports their caregivers who may find it hard to manage their loved one’s changing needs during this period.

This is for clients who:

• Have been diagnosed by a doctor to have a serious illness with a prognosis of less than a year

• Do not require 24-hour nursing assistance
