I am residing overseas and unable to visit an MOH-accredited severe disability assessor in Singapore. Can I see my overseas doctor for the required assessment?

If you can, we encourage you to return to Singapore for your SDA. However, if it is not possible, you may see your overseas doctor to complete the assessment using the Overseas Severe Disability Assessment Form. Please email apply@aic.sg to obtain the form.
