Can I apply for rollator under Seniors’ Enabling and Mobility Fund (SMF)?

Seniors currently undergoing therapy sessions with qualified therapist(s) in the restructured hospitals, community hospitals or other participating MOH-funded community-based care providers can approach their therapist to put up a SMF application for them. Otherwise, you may approach the Senior's doctor for a referral to a therapist to seek help to put up the application for a suitable mobility device under SMF.


If the senior is not attending any therapy sessions and is not on any follow up at the restructured hospital, you may fill in the SMF application form and submit to us together with the following documents:


A copy of Senior's NRIC (Front and back)

Medical reports (if available)

Financial Assistance documents (if any)

All forms and supporting documents should be sent to AIC via mail:

Upon receipt of the documents, we will process the application.

If the senior has not been means-tested in the past two years or if there are changes to the address, household income per person or household members, please submit the Means Test Declaration Form with relevant supporting documents to the following address:
