Introduction to Community Health Centres

Community Health Centres​ (CHCs) work with General Practitioners (GPs) to take care of patients with chronic illnesses like diabetes.

Together, both GPs and community health centres keep an eye out for the early signs of disease. Taking action quickly can protect you before a disease worsens.​​​​​​​​​​​​​

The Process

For better coordination and continuity of care, you need be referred by a GP, who will make appointments with the nearest CHC. Test results will be sent to the referring GPs for review and to follow-up with you. All services are by appointment only.
