What if I forget to bring/have lost my NRIC, PG/CHAS/PA card or existing assistive devices on the screening day?
If you forgot to bring / lost your:
1. NRIC – It is important to bring along NRIC. If you have lost it, you may use other form of identification to proof that you are Singaporean aged 60 and above. Seniors without any identification might not be able to participate in the screening.
2. PG/CHAS/PA card – You may still go for functional screening but you might not be entitled to certain subsidies reflected in the table above (refer to Qn 7). Note: This applies to expired PG/CHAS/PA cards too.
3. Existing spectacles, hearing aids and/or dentures – You may still participate in functional screening.
