If your loved one meets all the eligibility criteria for a nursing home, you may approach the Medical Social Work Department at the hospital/ polyclinic where your loved one is receiving treatment or goes for medical appointments at. The Medical Social Worker (MSW) will assess him/her and if found eligible, the MSW will assist to put up an application to the Agency for Integrated Care (AIC) for him/her.
