How can I get my functional disability status assessed?
Any registered doctor under Singapore Medical Council (SMC), Registered Nurse under Singapore Nursing Board (SNB), and registered physiotherapist or occupational therapist under Allied Health Professions Council (AHPC) can conduct the functional assessment. You may call your regular doctor to check if he/ she can help you to perform your functional assessment.
You can also visit your neighbourhood clinics where the doctors have expressed their willingness to assist the patients to perform functional assessment.
When you visit the doctor for a functional assessment, please bring along a copy of the Functional Assessment Report form (this can be downloaded from here.) The doctor will assist to fill up the Functional Assessment Report form accordingly, and return the completed Functional Assessment Report to you for your submission to AIC.
If you are currently receiving any Community Care services (e.g. Home Care Services, Centre-Based Services) from a MOH-subsidised service provider, you may contact your provider regarding the functional assessment. They will be able to facilitate your assessment.
