How can I get my functional disability status assessed?
For disability assessment,
This must be done by a General Practitioner (GP) or an approved healthcare professional using a Functional Assessment Report (FAR).
You will need to pay the assessment fee, if any. The cost may vary so do check with your healthcare professional.
For care recipients with Autism Spectrum Disorder (ASD) or Intellectual Disability (ID)His/ her needs will be determined by a Ministry of Social and Family Development (MSF)-approved assessor using a Client Assessment Form (CAF).

For those currently applying for severe disability schemesYou do not need to complete the FAR if you are currently applying for:
MediSave Care
The MOH-accredited severe disability assessor who will need to do the disability assessment for these schemes can do the disability assessment. This way, you do not need to pay 2 separate assessment fees.
If you are currently receiving any Community Care services (e.g. Home Care Services, Centre-Based Services) from a MOH-subsidised service provider, you may contact your provider regarding the functional assessment. They will be able to facilitate your assessment.
