Care recipient's nationality

He/ she must be a:

• Singapore Citizen
  OR

• PR (with a parent, child or spouse who is a living Singapore Citizen).

Care recipient's residence

He/ she must:

• Be living in Singapore.

• Not be living in a residential long-term care institution.

Care recipient's household income

He/ she must meet the means-test criteria for HCG:

• Household monthly income per person must be $4,800 or less.

  • Find out how to update your household details for means-testing

OR

• Annual value (AV) of property under $21,000 if there is no household income.

Care recipient's needs

He/ she must:

• Permanently requires some assistance with at least three of the six activities of daily living. The six activities are:

  

• Undergo disability assessment.

  • This must be done by a General Practitioner (GP) or an approved healthcare professional using a Functional Assessment Report (FAR).

  • Find out how to book an appointment to do the FAR.

  • You will need to pay the assessment fee, if any. The cost may vary so do check with your healthcare professional.
    
    For care recipients with Autism Spectrum Disorder (ASD) or Intellectual Disability (ID)

  • His/ her needs will be determined by a Ministry of Social and Family Development (MSF)-approved assessor using a Client Assessment Form (CAF).

    For those currently applying for severe disability schemes

  • You do not need to complete the FAR if you are currently applying for:

    • IDAPE

    • ElderFund

    • ElderShield

    • CareShield Life

    • MediSave Care
      
      The severe disability assessment done by the MOH-accredited severe disability assessor is enough to assess your level of disability for HCG. This way, you do not need to pay 2 separate assessment fees.
