Talk to your doctor at the hospital or polyclinic. They can help submit the application online.

You may also visit an AIC Link to get help with the application.

Cost

The cost of each round trip starts from $40, before any government subsidies. The final cost depends on the type of service used (eg transport only, or both escort and transport)  

To qualify for subsidies, your loved one will need to be a Singapore Citizen or PR. Subsidies are also means-tested (ie there is an income criteria).
