How do I apply for the Seniors’ Mobility and Enabling Fund (SMF) Assistive Devices subsidies?

There are several ways you can apply for SMF:
 

  1. If your loved one is receiving care at any public or community hospital, polyclinics, day rehabilitation centre, or senior care centre

    You can work directly with the therapist or medical social worker to apply for SMF. 

  2. Apply online

    You can do so using Singpass

    If you do not have Singpass, you can apply online here

    We strongly encourage you to complete and submit the application form online via your Singpass.

  3. Apply via a hardcopy application form

    You can download a copy of the application form, or get it from any AIC Link.

    Mail the completed application form to:
    Seniors’ Mobility and Enabling Fund (SMF)
    No. 7 Maxwell Road #04-01
    MND Complex Annex B
    Singapore 069111

    We seek your understanding that hardcopy applications take a longer time to process. 
     
    Documents needed

     

    Besides filling up the relevant application form above, you will also need to provide:

     

    • Your loved one’s NRIC (front and back), or a copy of the Digital NRIC available on SingPass 

    • Financial assistance documents if applicable (eg valid ComCare Long Term Assistance Card, Medifund, etc)
       

    For motorised device applications
     

    • Medical reports (eg discharge summary or doctor’s memo).

     

    If the applicant does not have a doctor’s memo, you can download this Certificate of Medical Need Form and get your doctor to complete it.

    For incomplete applications

    Please use this form to submit the missing documents as stated in the letter.
