Advance Care Planning (ACP) is for everybody, regardless of age or health condition. You are strongly encouraged to have these conversations with your loved ones as early as possible. Having your wishes documented in an ACP can also ensure that the medical team can be guided by your ACP in the future. ACP also gives your loved ones assurance and confidence to make medical decisions that are aligned with your wishes during a medical emergency.
