Can I use an old medical report to complete my PR formalities?

Generally, successful PR applicants* (aged 15 years old and above) are required to submit a new medical examination report
 
However, if the applicant had submitted a medical report (with both TB and HIV screening results) to ICA or MOM not more than 2 years ago from the appointment date to complete PR formalities, he/she may complete the declaration portion of the medical examination report. 
 
*Note for P1, P2 and PEP pass holders only: Please refer to this medical examination report instead for your PR completion of formalities.
