Can I submit a hardcopy Lasting Power of Attorney (LPA) application?

From 14 November 2022, all LPA applications must be submitted online by default. If you're uncomfortable with technology and need help completing your LPA application online, visit a CitizenConnect Centre or ServiceSG centre (list on OPG website) or contact the Office of the Public Guardian (OPG) hotline at 1800 111 2222. OPG may accept hardcopy LPA applications if you fall within one of the following exceptional situations, subject to the Public Guardian’s approval:

  • you or your Donees (including Replacement Donee) are unable to complete the application online due to physical disability or other circumstances

  • when both the prepare your LPA service on My Legacy and the Office of the Public Guardian (OPGO) are down

  • you do not have an electronic device to receive SMSes or create and access the Singpass account

  • you or your Donees (including Replacement Donee) are not eligible for a Singpass account


If one of these situations describes yours, you may submit a hardcopy LPA application. Download and print the LPA Form from OPG website, complete it, have it certified by a certificate issuer and mail in or drop it off to the Office of the Public Guardian (OPG) at 20 Lengkok Bahru #04-02, Family@Enabling Village, Singapore 159053. Please include a cover letter to explain why you’re submitting a hardcopy application instead of an online application.
