What are the documents required for submitting medical monitoring reports?

The following are the documents required for the submission of various medical monitoring reports:

Summary report of examinations and register of employees for exposure to a hazard:

  • Completed summary report of examinations
  • For noise hazard only, results of abnormal audiograms for noise hazard
  • For other hazards, register of employees and copies of all the test results.

Certificate of suspension:

  • Workplace number of the company where the affected worker is employed
  • Details of the affected worker including name, NRIC or FIN, occupation and department
  • Copies of test results

Certificate of fitness:

  • Workplace number of the company where the affected worker is employed
  • Details of the affected worker including name, NRIC or FIN, occupation and department
  • Copies of test results
