If your caregiver has enrolled you via the HealthHub app, he/she will be able to book an appointment for you through the same app.
Alternatively, you may approach the nearest Enrolment Station for assistance with the appointment booking. Healthier SG ambassadors at the Enrolment Station will provide you with an appointment card to bring along for your appointment. Subsequently, you will receive a Health Plan Booklet at your Healthier SG clinic to keep track of your appointments and guide you towards achieving your health goals.
