You do not have to top up your MediSave Account when you withdraw your CPF savings from age 55. The Basic Healthcare Sum (BHS) is the estimated savings you need in your MediSave Account for your basic subsidised healthcare needs in old age. It is the maximum amount you can have in your MediSave Account.
